Video Intake Form

This form helps us understand what you want to communicate, who needs to hear it, and what the project will require. Our goal is to make the video process as smooth and effective as possible and, most importantly, to tell your story and share your message in the strongest way we can.

Video Production focuses on filming, editing, and the technical side of bringing the video together. To help the project move smoothly, the requestor provides the content expertise, coordinates participants and locations, supplies required assets and permissions, and manages internal approvals and consolidated feedback.

We know this is a lot of information, and you may not have every answer right now. That’s okay. Complete what you can, and we can work through anything still open during a production meeting. After submitting this form, please schedule a production meeting with Jeff Allen. Jeff keeps his calendar up to date, so feel free to choose an available time that works for you.

If you would like the video considered for publication on Easterseals Midwest’s YouTube channel, please indicate that below. Final decisions regarding YouTube publication will be made by the Marketing & Communications team.

5. Production and Filming Needs

Could VocalVideo work for this project?

Easterseals Midwest can collect video directly from participants using VocalVideo. Participants receive a link and can record from a phone, tablet, or computer using their camera. They can respond to one or more questions or record a prepared/scripted message, then submit the recording for use in the final video.

VocalVideo can be a good option for projects such as employee testimonials, internal announcements, training content, messages from participants in multiple locations, or other projects that do not require an in-person video shoot.

Standard graphics may include titles, lower thirds, logos, animated text, or basic chart/graph animation.

Projects requiring custom illustration, character animation, advanced motion graphics, complex animated sequences, or other specialized animation will require an outside vendor and separate budget and may affect the project timeline.

Will existing photos, video clips, logos, graphics, statistics, documents, or other materials be used in this project?

If not, that’s completely fine. Video Production can use the style and approach typically used for Easterseals Midwest videos.

Final content accuracy should be confirmed by the requestor and designated reviewers before approval.
